1) For minor omissions or errors, the user can return to a note that has been completed and signed and make an addendum that will have its own time, date and author stamp:

2) Since an addendum will not be reflected in later pre-populating models, it is sometime more appropriate to address a significant error or omission by re-writing the note that contained the error and marking it as "Non-billable" (if it is a billable note). Given that most notes will prepopulate from the last note written, this is not as big a job as it might seem and it will correct the record for notes that will be populated by that data down the line:

3) Finally, in cases where it is warranted, we can provide an agency with a path that would allow the agency to unsign even a completed and signed note, which would allow the author to either edit the original or delete the note altogether. Because this feature can easily undermine the integrity of the agency's medical records if used too widely (especially if the feature is used to remove notes for services for which the agency has already been paid), we grant it to one or a few designated users in the agency only. Once a user has completed the process of unsigning a note, the author of the note can go back in and modify the note or remove the note as needed.

To note: Once a note has been unsigned, only the original author can delete the unsigned note.
