• To create Client portal forms you will need to go to Dashboard>Client Forms & Portal>Client Portal Forms.

• Click the plus symbol to create a portal form.

• Fill in the title of the form, when and how often it needs to be completed, add any instructions you would like for your clients to complete the form and check the programs you want the form to appear in for your clients.

• There are several Pre-built model pages that are available in the client portal you can use to gather information in your forms that you can select by clicking this plus symbol and choosing the pages you would like to add to your portal form

• To create portal forms with custom information for the purpose of gathering information from clients specific to your program via the client portal, you will need to make sure you have client models also know as custom models designated to appear in client portal.

• You may create custom models for the client portal or you may designate current custom models you have created by checking the checkbox in the edit custom model box at the top of your custom models

Once you are done adding your pre-built models and your custom client models click the save button and you are done.
