This report uses the Substance Use Assessment model. This models allows the creation of a substance use assessment for a number of substances. For each substance, you can add one or more evaluations. The Substance Use Assessment History report, shows client information along with substance information along with evaluation information. Pick this report from the Client list.

Once you pick the report you will be asked for filter parameters. You can filter on Date Of Service of the Evaluation, the program, and/or the location.

The report will show one line per evaluation date of service.
